Depends: It is no longer needed when it will no longer be beneficial. If you have a 45yr old woman with metastatic lung cancer it would no longer be recommended to get a mammogram. A healthy 85 yr old woman shoul. Generally if life expectancy is 5years or more it would be recommended to get a mamogram.

Never: Mammography yearly is advised for all women over 40 or 50yrs. Breast cancer can occur at any age.

Judgement call: When is stop is a judgement call in my opinion. The purpose of a screening test is to identify cancer early, save lives/extend lives. But how much life is there left to save, can vary in the geriatric population. A bedbound 70 year old with multiple diseases, a mammo wont add much. But an 80 year old, running half marathons, that person has a long way to go and a mammo could save her life.

It depends: There is no clear cut answer. If you are in relatively good health, no matter what your age, you should probably continue screening. If you have a terminal medical condition, it may not provide much benefit to continue screening.
