Endo pain: Endometriosis related pain could be secondary to proinflammatory substances in the peritoneum. This substances irritate the nerve endings in the pelvic organs. Could be due fibrosis around tissues that then have decrease in blood flow causing pain.

Inflammation: Endometriosis makes inflammatory chemicals by its local immune response. Endometriosis tissue also make new and sensitive nerve endings that can magnify he pain signal. That's why the intensity of pain and amount of disease don't correlate well.
