Unlikely: There are dozens of health care insurers across the country.It is doubtful that all will be represented in any state, or that all will offer each level of plan specified by the elaborate requirements of the program.At the present time the lowest cost plans may also have substantial up front co-pays and out of pocket deductables that will be surprising to those looking for the promised "great" fix.

Will vary by state: It will depend on which companies decide to sell health insurance in a particular state. The type of plan and level coverage that an insurance company sells through the obamacare health insurance marketplaces should be the same quality plan as the company sells to people outside of the marketplaces. There will be different levels of coverage, with probably higher premiums for broader coverage.
