Fungal infection: Toenail fungus is an infection of the bed and plate underlying the surface of the nail. You should treat toenail fungus as soon as it appears.  Yellow toenails are a side effect of the presence of toenail fungus. The more discolored the nail, the further along the infection.There are many antifungal agents in the market to use from.

Could be : Fungus. If a small amount isn't sent for KOH test to pathology lab there is no way of knowing. See if dr. Ran this test or not.

Fungus is : Usually the culprit, though bacterial colonization and subungual keratinazation can be other causes.  A sample of the nail send to a pathologically lab where special stains are done can help differentiate the three, so that one would know how to treat.

Split perso-nail-ity: This sounds like a nail root phenomena. The root appears to be making nails with possibly  a split layer of nail making cells. To remove all doubt, see a specialist with knowledge of how nails grow.

Not usually: For most patients, nail fungus is primarily a cosmetic issue. Some people, like those with diabetes, it can lead to bigger health problems. If the problem is very limited, topical treatments may still work. If the nail is affected to the very base, treatments become more involved.

Diabetes & Wt Gain: Type 2 diabetes is a condition of Insulin resistance meaning your body makes Insulin but it does not work as well.  As a result the body needs higher amount of Insulin to control your blood sugar.  Unfortunately, Insulin also stimulates fat formation, epecially the unhealthy fat around your waist.  The result is weight gain.  High blood sugar also promotes the growth of fungus causing yellow nails.

Osteomyelitis: Is an infection in the bone. If you are able to see pus coming from bone, i think you should be in the hospital being treated. Pus from bone needs to be drained properly. You should be on antibiotics as well. This is a serious problem which may require bone to be removed.
