Get an exam: If you see something swell and go down again when you bear down, you need a thorough testicular exam and hernia check. Those are more likely than a communicating hydrocele at your age. There are other more benign possibilities, but those are the most likely.

Probably a hernia: A communication hydrocele is an indirect inguinal hernia by definition. While the testicle could be swelling from an inflammatory process, it is less likely than a hernia. See your doctor for an exam. May need a referral to a general surgeon as well.

Unlikely: It may be a hematoma (like a bruise, but around the testicle instead of under the skin), or there may be greater damage there.  In any case, it should be seen by your doctor asap to make sure there is no serious damage there.

Yes: hydrocele surgery is a simple  and takes care of the issue.  It is usually an outpatient procedure like a vasectomy.  Discuss side effects with your surgeon.  These would usually be reactions to medication and possible complication with bleeding.

It is hard to tell: It is hard to predict if it will go away.  Often they do not go away. See the surgeon or a urologist is it is painful or too big and bothers you. Usually they are not harmful.

Yes: Swelling that is tense and round is usually a hydrocele.  Hernias are more ill-defined, and may be very large, and sometimes painful.  Either one may occur on one or both sides.  If the swelling comes on suddenly, it is more likely to be a hernia.

Reevaluation by surgeon: Typically a hydrocele hernia defect if it is an indirect hernia with closure of the sack with or without mesh will resolve the condition.
If the sack is still open you may see variation in size. However this may be postoperative swelling. A return for evaluation with your surgeon who performed the procedure should be your first step.

?Hydrocele of cord: Disappears with surgical removal. Groin lump after which develops denovo after hydrocele surgery could be due to a bleed related to the surgery, post operative wound infection or enlarged groin lymph node/s. Consult your urologist either way.
