Diabetes: Uncontrolled diabetes very often causes nerve damage, especially in the feet. Over time the high sugar in the blood causes damage to the tiny blood vessels that feed the nerves and then they are affected. Other complications from diabetes involve losing vision, kidney failure, heart disease. There are many treatments for diabetes that should help. Go to the doctor for an evaluation and treatment.

Legs: He may get a benefit from taking trental (pentoxifylline). See your dermatologist to find out the best treatment.

Numb feeling in leg: He may be developing a possible blood clot in his leg or parasthesias secondary to his diabetes. See a doctor asap.

Heel : Then see another podiatrist .  I get the idea that you are condemning all podiatrists because you did not get relief .  Seek another opinion.  This could be vascular , stress fracture, etc.  Go get the help you need

Have it looked at: If it is only on one foot then there is a chance he could have a blood clot in that leg and should be looked at today. Swelling in one foot and numbness that came on all of a sudden shouldnt wait. Good luck.

No: Back and leg problems are common in diabetics.  Kidney problems can occur with diabetes but are usually silent (no symptoms) and must be checked out by regular medical and laboratory testing.

Lumbar radiculopathy: or lumbar disc/nerve root irritation or compression. Get seen adap. You may need nerve conduction studueus and an MRI of your lumbar spine.

You need to see: Yur doctor for further evaluation. It is possible you have a vitamin/mineral deficiency. It is not uncommon for a diabetic to have low magnesium in their red blood cells. This couldbe causing your spasms, the falling might be due to bad balance from neuropathy. Consult with doc for definitive diagnosis and treatment.

Too many factors: I have seen gas gangrene develop in 18 hours.  Other gangrene can take longer.  Give me more info.

Pain: Two things:
1: You need a referral to a Neurologist, and soon!
2: I simply don't like the term "type 2 diabetic". No you are not "diabetic" - you have "diabetes" and you should try and get rid of it.  Not always possible but you should see a diabetes educator that would help you in the right direction.
