There are lots of: Potential causes. On thought that comes to mind is to make sure you don't have diabetes. People with diabetes can have what is called neuropathy from deposition of the sugar onto the nerves and its resultant damage. Best to get a full physical exam and routine labs with your internist. Stop Playing around.: Sounds like there's a whole lot going on here. Any one of the areas of pain could be the problem and compensation causing the rest. 
Could be soft tissue or bone or both. Your best bet is to have it examined to eliminate the possibilities. My guess is that you will need a cam walker of stabilize the foot for a while.

mri neg., fracture boot for 4 weeks, refrained from activity 6 weeks, still have top of foot pain.  what now?

Have you tried thera: Could be tendonitis which may need anti-inflammatory meds and physical therapy.  Have an MRI to determine what cause is.
