Hydrocele or tumor: Hydroceles are a collection of fluid around testicle ; commonly first thought, by subject, that testicle inself is enlarged. Outline is smooth and feels like a tense fluid filled sac. Testis tumors are hard ; irregular. Leukemia, or lymphoma of testis can be a hard, smooth enlargement. Scrotal ultrasound is an excellent, non-invasive diagnostic tool for enlarged testes.

Multiple causes: You need to be examined by an experienced practitioner. Your problem could be anything from a testicular growth to a hydrocoele (collection of fluid) to a hernia to an abscess.

Dull ache: can occur;however, if not performed recently, get a scrotal ultrasound  and a urology consultation

Checked yes: Unlikely an emergency, and unlikely to be a malignancy. Give him a couple of weeks to have it checked out by his primary care dr. Or a urologist.

Scrotal cysts : They sound like benign scrotal cysts. These are clogged pores that form cysts which contain sloughed skin cells. See a urologist or dermatologist to have them evaluated and removed if needed.

i had varicocele surgery for grade 4 and it been 45days i still have larger veins, and also my testicles hang lower than usual is this normal? do my larger veins disappear and my hanging issue get recover?

: The veins generally will reduce in size over time.  The veins should not feel full or rigid after the surgery, however.  If it seems that the veins are the same as they were prior to the procedure there may have been an incomplete ligation.  See the surgeon for reevaluation.

It is slightly difficult to fully diagnose these lesions without seeing it.
It can be normal.
