Urinary symptoms: First, make sure you don't have a urinary tract infection by seeing your doctor. 
Having another person in bed with you is difficult; you may not want to get up to urinate and wake your sister so your bladder overflows and causes the leakage at night. 
It sounds as though there may possibly be some complicated family dynamics. Talking to a doctor about these issues may be very helpful.

Bed pillows can easily entrap and smother an infant or small child. It is also best to wait until child has graduated from crib to toddler bed where movement is not as restricted. A firm and small (airline-sized) pillow is best for a toddler.

f is for face: facial droop or crooked smile. A is for arms: inability to move arms or one arm being stronger than the other. S is for speech: can your loved one repeat a simple sentence or is their speech slurred? And t is for time: if you see any of thse signs call 911 as soon as possible.
If your wife is confused and not making any sense, this is of high concern and needs to be taken to the emergency room. If you suspect stroke, call 911 for timely treatment. Time is brain.

It's found in ~10% of folks with tourette syndrome; however, the diagnosis requires multiple motor tics & at least 1 vocal tic for a year or > with no  tic-free periods > 2 mos. Adhd & ocd often co-occur. Her pediatrician may consult a child neurologist or psychiatrist or a developmental/behavioral pediatrician; she needs help asap. See tsa.Org.
