See physician: It needs to be determined for sure that this is a fungus as opposed to something else: ie, trauma, psoriasis or other condition. Affected portion of nail may need to be tested to help make diagnosis. If it is a fungus, there are different typed of topical and oral medication. Consult with podiatrist.

Plantar warts: Various strains of the common skin wart can infect that location &amp; lead to a painful thickened area. Removal by a dermatologist or podiatrist may be your best option since they tend to grow larger over time. Your doc can confirm/exclude this label.

Down under the toe.: It sounds as though you've managed to injure the toe. Bruising is a sign of underlying tissue damage and steps should be taken to minimize further damage. Exercising should be eliminated until the effects of the injury are under control. Ice compression, elevation and rest are the mainstays of treatment. To remove all doubt, have the toe x-rayed and examined to assess the  degree of injury.

If doc suspects some unusual issue it is best that s/he be up front about it and do appropriate screening or specific tests to define it.Proper diagnosis can either relieve you or make sure early treatments are started.

Now it depends on your sign and symptoms. Why you were referred for mri?
You need to see a neurologist.
