i had pain under the left shoulder blade imtermittently.now when i got covid it is very annoying(it happens when sleeping on my back or on my left side), when sleeping on my right shoulder or standing no pain.can it be organ related or muscoskeletal?

: It is most likely to be musculoskeletal,if there are no other symptoms. A heating pad and ibuprofen may provide relief. Watch for signs of shortness of breath, prolonged fatigue and cough.

Sounds: Muscular. The CT would have identified a thoracic disc. Most organ pathology would be negated. Try NSAIDs and light torso exercise!

For how long?: Acute pain that is persistent can be caused by any number of causes such as a muscle sprain or inflammation of tendons(Tendinitis).
If breathing makes the pain worse, then it points to your lung(Pleurisy?) or a broken rib from injury.
If the pain does not settle down with Tylenol(take 2 tablets 6 hours apart for 24 hours), then you need to get a medical examination and a Chest X-R ay fm your docto

Possibly: usually, gallbladder or aortic pain would not feel worse with pressure on the scapula. Have you been doing any physical activity? Sometimes, certain exercises and activities strain the rhomboids and infraspinatus muscles causing similar pain. If you're not getting better, get checked out.

Various causes: If there has been no recent injury or strain, possibilities include scapula or rib pathology, lung pathology, early shingles(watch for rash), or nerve impingement from a bulging disc or spur pressing on a nerve root at the lower neck intervertebral space. See your MD for evaluation, xrays and/or MRI.

Commonly missed: Rib: Several possible causes including pleurisy or lung injury.  Should see dr, asap.  But the most common cause for pain in this location which is not serious, but is extremely painful is a dysfunctional rib. Md's almost never make this diagnosis, but osteopathic docs who commonly do hands-on diagnosis and treatment, in addition to doing all the other work-up, often find and treat this problem.

More info?: Did you injure yourself?  How long does it last? Anything make it better?  Anything make it worse?  Does food affect the pain?  Have you had any surgery ? How long has it been there?  Have you tried any medications? Did they work?  Any nausea? Fevers?  If this is recent you could try ibuprofen, heat,  massage.  If long standing or worsening see a doctor to begin workup.
