Various causes: If there has been no recent injury or strain, possibilities include scapula or rib pathology, lung pathology, early shingles(watch for rash), or nerve impingement from a bulging disc or spur pressing on a nerve root at the lower neck intervertebral space. See your MD for evaluation, xrays and/or MRI.
