Left abdominal pain: Ovarian cysts are common if one is not on oral birth control, and the pain associated with it usually stops after the cyst ruptures.  See your healthcare provider for an exam and ultrasound if the pain continues.  Ovarian cancer is very unlikely to occur at age 20.

It depends: Fat on the ovary depends on location and how the fat is identified.  If fat is seen by imaging or by the surgeon to be attached to the surface, it most likely will be an adhesion with the omentum or abdominal wall. If fat is seen inside the ovary on imaging, it may indicate a teratoma (dermoid cyst), most often a benign lesion. Pathologic evaluation is necessary to exclude a lesion in the ovary.

In ovary  benign cystadenoma  may turn to malignant cystadenocarcinoma.

Not so likely: Women with polycystic ovaries are at increased risk for endometrial cancer because of the hormonal chaos. The cysts of the ovaries in polycystic disease aren't undergoing rapid cell turnover, hence the low cancer risk. By contrast, primary cancers announce themselves as individual cysts but grow rapidly.
