Office, if lucky: Hopefully your doctor's office has maintained his or her medical records and you could request a copy from them.

That said, depending on the doctor's office location, they may only be required to keep the records for 7 years (not 10), and may have shredded them.

But most offices keep copies longer than the legal requirement so you may be in luck.

Immunization records: Try calling your health department. I am uncertain about texas, but in new york, all vaccines given to a child is reported via computer to the state.

Contact society: Physicians that retire often transfer their records to another physician or set up a way for patients to obtain such information.A call to the local medical society can often supply you with the contact data. The state medical board may also have contact information.

Depends on situation: Many state require a physician to keep records for at least 7yrs after the patients last visit & their 21st birthday, whichever is later.Then they are destroyed. At retirement, many drs arrange for another dr to retain custody of their charts. You can call a local medical society & find out. Some states have a computerized verification system & you can access records thru a school.

Low priority now: HIB or haemophilis influenza type B was the primary cause of bacterial meningitis in the 0-4 year old age group prior to wide use of the vaccine (early 90's). It rarely struck older kids, and by their teens it was rarely if ever seen. You might find someone who would give you the series,but at this point, I would see little if any need.

Depends: Doctors records & school records are some of the best sources for early vaccine records. Doctors will eventually destroy old  records based on guidelines provided by state medical boards.  Some states have a statewide registry but you often have to sign up to have your data tracked.

i can't find my records of if i've had my second dose of varicella vaccine or not. if i go ahead and get one just in case, would a third dose be ok?

Vaccine dosing: Extra doses of vaccine aren't harmful. No problem to be revaccinated if uncertain.

is it safe to get the tdap vaccine if the last one was 5 years ago? i thought it was over 21 years but then double checked my records and it was from 2016. should i be worried?

: No need to worry.  If you had a high risk injury, receiving a dose of TDaP to reduce the risk of tetanus would be suggested.  At 5 years you should be entirely fine

is it okay to get the mmr vaccine a second time? i think i may have gotten it when i was really young but i have no way to track it or find any record of it. i am now 25 years old and need immunizations for college?

Yes: In the USA it is routinely given at age 1 yr with a booster at 4-5 yrs and sometimes again in women who show poor antibody levels prior to planned pregnancies. Repeating the vaccine does not place you at any  specific risk.

Doctor or school: Check with your doctor first.  In today's medical world most old records are scanned in and kept for years (6-25 years after your last visit), depending on the state).  If your doctor retired/died, check with the state medical association.  They should know who has those records.
If you've been in school recently then your school will have a copy.  Not sure how long they keep them though.
