Few hours: Penicillin starts to work as long as it is absorped if given orally that is within few hours and keep working and peaks somewhere after 5 days, it is usually given for 10 days course
but it starts to work a little earlier if given by intramuscular route and immidiately if given intravenously.

RF prophylaxis: Penicillin for Rheumatic Fever prophylaxis is due for several years after the acute episode,which you probably sustained in your childhood, and you already developed a rheumatic heart.Daily penicillin is used,not practical,that's why long acting penicillin injections are used every month for about 10 years,or till age 25 years, whichever is longer,depending on the type of heart problem,see comment

Also depends upon what the pcn is treating. If strep throat, never. Pcn begins to inhibit the growth of bacteria and then the wbcs help finish the job.  This is complex.

10 days: Treatment is usually for 10 days. The adult dose is 500 mg  three times/day. Take it as prescribed, unless side effects.
