: PPI can cause both acute and chronic kidney disease. The acute injury is in the category of allergic reaction, called acute interstitial nephritis and it may happen weeks or months after the exposure. PPI-related chronic kidney disease occurs after a prolonged and continuous use of PPI (many years of exposure). The definite physiopathology of the PPI-related CKD is still not completely defined.

Magnesium level: There is a blood test for mg but since most mg in the body is in tissues and not in the blood, blood tests may not detect mg deficiency which is avoided with a diet rich in nuts, leafy vegetables and whole grains or by taking a mg supplement. Ppi's shouldn"t impact your mg level.

Don'tWorryBeDiligent: GERD is common among American adults but only a small percent of people with GERD(less than one out of every 10)develop Barrett's esophagus.Between 0.1% to 2% of those with Barrett's esophagus each year go on to develop esophageal cancer.So you have time to work it all out &amp; there are several ways to treat it if it progresses.Key to all this is prevention &amp; close follow-up by a Gastroenterologist.

Depends on dx: Domperidone and metaclopramide are pro-kinetic agents, that help stimulate stomach and intestinal motility.  If you are having gastroparesis as the underlying cause of reflux, then you may also be a candidate for gastric pacemaker.  This involves significant screening and testing to determine whether you would be a candidate.

Yes: Need to asses to need  for long term therapy and no long term therapy should be left without an endoscopy at some point to dertemine the etiology of the disease causing long term use. Gerd with barretts or chronic dyspepsia all can be a cuse for long term ppi. Downgrading to an antihistamine suce as Zantac (ranitidine) is appropriate as well you should always consult you doctor  whenever you use long term meds.

drs say that ppis should not be take long term, so what does someone do, when diet and lifestyle measures dont treat their symptoms? like what am i supposed to do? i have been on for five weeks ppi and pepcid. i stopped pepcid and i feel like i did?

: In general, PPI should be stopped after GERD symptoms are under control because of PPI' side effects. However, for people with severe GERD, they have to take PPI in the long term or a histamine 2 blocker such as pepcid can be tried as a substitute because of less side effects in the long term. 50% of my patients can't get off PPI. If GERD is due to severe hiatal hernia, fixing that can spare PPI.

i've been prescribed antibiotics for 1week and ppis for 2weeks for h.pylori. shouldn't it be the same? do i take both for 1 week or as prescribed?

Reasonable: Without knowing which medications you were prescribed, there is supportive data looking at a course of 7 day therapy with lansoprazole (prevacid), amoxicillin and azithromycin for eradicating H. pylori.  The standard had been to use 10-14 days; certainly over the shorter courses .e.g 5 days as  there were better results.  I suspect that the PPi is for longer on account of allowing healing of ulcer
