See doctor: Several viruses can cause fever with skin rash, and any child with fever and rash should be seen by a doctor, preferably the child's pediatrician. Mostly these are mild but sometimes serious. Better safe than sorry. Good luck.

Earache-wait?: Hard question.  Pain is your body's way of saying that ain't right.  Generally i would say two days, you should be seen.  Barring obvious signs of serious illness and a contraindication to using decongestants, afrin nasal spray, otc antihistamines like Claritin or zyrtec and valsalva maneuvers (popping ear like you coming down a mountain) may do the trick. Don't forget teeth can cause ear pain.

Depends: Younger children can tolerate fairly high fevers; i would be concerned anytime the cause of the fever is uncertain. Otherwise temps up to 104 can occur with relatively minor infections like colds, flu, etc. If the child is lethargic, repeatedly vomiting, seizing, having difficulty breathing, in significant pain, etc. Should be seen by md.

Keep  a close watch: You've done the basics having your physician involved and that goes a long way when you're dealing with fever. If her initial evaluation pointed to viral rather than a bacterial infection, we just wait it out. Some viral illnesses are marked by 3-7 days of high fever followed by a rash. As long as she is taking fluids well & remaining playful at times i'd monitor & recheck if needed.

Not unexpected: Low grade fever is a common finding in the 2-3 days after vaccinations. It is a good sign the kids immune system is active in recognizing the vaccines. It is not a sign of a bad side effect. You can call if you wish but this is relatively normal. Mention it at the next regular visit. Persistence beyond 3-4 days would be unusual.

What else ? : Many simple self healing viruses present with such symptoms. If the kid is able to take in meals &amp; fluids well enough to stay hydrated, plays some during the day and gets good rest, you may be able to monitor it at home. If she lays around doing nothing, has no appetite &amp; won't drink, or otherwise spooks you I would take her in.
