by lmp i am 7w4d today. u/s 2 weeks ago told me turning 5w. only sac, baby barely visible. today, fetal pole and hb but baby measuring 5w6d. problem?

First trimester date: Depends on what the heart rate is. Assuming a normal heart rate, it is likely just that the ultrasound is more accurate at dating your pregnancy. Dating by LMP assumes a 28 day cycle with ovulation on day 14. Not all women have that cycle interval, making first trimester ultrasound more accurate in most cases.

no fetal pole visible at 22,000 hcg (5w3d). is this a problem?

Follow up needed: If you are really 5-6 weeks then one would not expect to see a fetal pole yet. A sonogram can be repeated in at least one week. BHCG levels vary widely and can not be used to draw definitive conclusions. That is a high number for only 5 weeks though.

Possibly: Do you know the actual measurements ?There is a guideline that can be used called the 5-10-20 Rule for Crown Rump Lengths and Gestational Sacs
&gt; if  you have 5mm CRL you should see heart beat, this should be by/at 6 weeks
&gt;if you have 10mm GS, you should see a yolk sac, if not then nonviable
&gt;if you have 20mm GS, you should see a fetal pole, if not then nonviable. Again these just guidelines.

around 5 wks preg. on jan. 6 hcg was 13,200 & jan 8 it was 23,000. had iu us on 8th. yolk sac visible, but no fetal pole. hcg too high for +outcome?

Too early to know...: At 5 weeks, you may only be able to see the yoke sac.  This doesn't mean something is wrong.  You don't always see a heart-beat until 6.5 to 7 weeks.  Remember that 5 weeks is only 3 weeks from conception.  Its very early.  I wouldn't worry at this point.  You might consider repeating the scan at 7 or 8 weeks.  Best wishes!

Doubtful: Usually when twin pregnancies occur fertilization happens at approximately the same time. The growth should be very consistent with each other. It appears that baby a will not continue to grow. I'm sorry.

Early embryo body: Its the embryo body usually seen as a small dot next to a small circle, the yolk sac,  during earliest stages of pregnancy. 
The first structure visible is the yolk sac.  The fetal pole is then seen as a small bump on the yolk sack wall.  If you think of a chicken's egg, it is the small white thickening next to the yolk that develops into the chicken. The yolk sac dissapears as the embryo grows.

Tough question...: I imagine you are aching to know if everything's ok with your pregnancy. While it is generally possible to see the heart beating at about that time, the problem may be dates: if you are three weeks rather than seven, for example, you won't see much on ultrasound.
