Baby teeth: Should all have fallen out 10 years ago.  Either missing or impacted permanent teeth.  Black toot may be cavity or resorption.  A Dentist should have pointed this out to you years ago when x-ray's were taken.  See a Dentist now.  Orthodontic and/or Prosthetic treatment may be in order.

The permanent tooth has a capsule around it the eats away at  the baby tooths root until there's so little root left, the baby tooth will fall out. The permanent tooth should start to come out in a few weeks.

Pediatric dentist : Did they come in discolored or did this happen after falling or  perpahps there is decay.   The best thing for you to do it take him to a pediatric dentist for an evaluation.

Generally No: The important thing is to make the experience fun for the child:'the tooth fairy is coming with a gift when that tooth comes out'. We all have wiggled are teeth out as children and we learn how to do it . Be positive and keep it light for the child.

Depends on age: Most children will lose a first tooth by age 6.  Most will have lost all baby (milk) teeth around age 10-12.  Sometimes the adult tooth above is not coming in at the correct angle to push out the baby tooth-- your child's dentist will help determine what, if anything, should be done.
