Baby teeth: Should all have fallen out 10 years ago. Either missing or impacted permanent teeth. Black toot may be cavity or resorption. A Dentist should have pointed this out to you years ago when x-ray's were taken. See a Dentist now. Orthodontic and/or Prosthetic treatment may be in order.

Primary teeth: You'll need to see a local dentist who will take radiographs to determine where exactly those secondary teeth are. That black tooth may indicate decay.

Dental exam: Your milk teeth or Baby teeth should have fallen out by now and permanent teeth erupted. A dentist can determine why this hasn't happened. Sometimes teeth get blocked from erupting, sometimes permanent teeth don't develop. As for the dark spots, this may be cavities, stain or calculus. A dentist can help you.

Contact DDS: Have your dentist do a definitive examination complete with x-rays to determine a treatment plan. When was the last time you had a through examination for decay and the status of any primary teeth? Once a treatment plan is decided upon, it can be assessed whether orthodontic therapy is needed in addition to possible implants.
