Recommend skin graft: Allowing to heal in on its own will take longer time. amputation without bone exposure not indicated. cross finger flap or thenar flap could be best option. Hard to say without visualizing the amputation. full thickness graft results in decreased sensation. Consult hand surgeon

If there is an : Issue with the outcome of a procedure that was done it would be wise to return as your doc has experience performing the procedure and handling outcomes. It may require something as easy as smoothing down the nail with a file and recommending a topical medication.

Toe bone injury.: It sounds as though that "mound of skin" may be the result of injuring the toe bone just under the nail bed. Occasionally, the bone, when injured responds by a sudden growth spurt. As it grows upward, it raises the nail bed up and can cause a nail distortion as you have described. An r-ray will usually confirm the event. The treatment involves removing the boney spur. Known as subungual exostosis.

Usually trauma: It is usually from some sort of trauma.  It could lead to an infection.  See your podiatrist soon.

Depends: If due to trauma, if separated portion of nail is cut away, it is possible that nail may grow back attached in area. Some times toenail can be removed with goal of having it grow back normally. If fungus has caused this or if nail root is damaged this will make it very difficult to reverse.

We don't always know: The why, just what to do. So i would keep them short. If there is any sign of fungus, i'd try and kill the fungus, .

Thin nail hydrate na: Thin the nail plate and hydrate the nail plate and nail bed with aquafor or similar ointment daily.  Take biotin 2000 - 3000mcg daily.
