Hard to tell: Perineum is the area between the vulva and anus(in females) or the base of the penis and the anus ( males). I guess somebody has to see it. Is it a tear? Is it after delivery of baby? Is there any skin disorder associated to it? Did you have any injury? Are you riding a bicycle?

Cyst: Sometimes an urgent care doctor can drain a cyst, especially if it is infected, and when it's important to have it drained soon. A dermatologist or general surgery clinic is also an option. Be extremely careful that this is not a peri-rectal abscess, since they are dangerous and life-threatening. See a doctor soon.

Prostate: The combination of perineal and penile pain often is due to low grade prostatitis.  See a urologist for a thorough prostate evaluation including a digital rectal exam. You may need cystoscopy as well.

Variable: Vagisil is over the counter and used in this manner. However, replens (used for menopausal vaginal thin skin) or something similar may help as well. If these do not work, see your doctor for other treatment.

See MD: If you are referring to your genetalia feeling swollen you should see your OB/GYN for a full history and exam. It would be helpful to know your age and medical history, sexual history. If you have a concern, you probably will benefit physically and mentally from a physician visit. Don't worry you'll be okay see your OB/GYN.

Needs to be seen: Lumps, bumps, blisters and other skin abnormalities on the genitals need to be evaluated. An examination will be needed to know the cause and treatment. Folliculitis is one possibility. Yeast is a possibility and over the counter fungal cream will fix it. Trauma from recent intercourse if possible. Herpes is an std which can cause this. Other possibilities too. Schedule an exam.
