Hard to say: Pains at night can be a normal result of activities during the day, and even arthritis which is commonly worse at night. Chest tightness is a concerning symptom, especially for a man in his mid 40s. It can suggest a serious problem like heart disease, or more commonly, reflux, or something like asthma. You have several things that may or may not relate. As you are suffering, best to get good exam.

: Joint pains that are worse in the evening are usually related to mechanical issues; that is, rest and then proper posture/position, muscle strengthening can help.  You will need to see a doctor to examine you when symptomatic to have a better idea.

Lupus?: This is extremely important; you need to see a rheumatologist or nephrologist to see if you have lupus, which and cause severe kidney damage. Lupus is treatable BUT you need to be seen urgently. Insist on getting an appointment with one of these specialists; make your PCP listen to you, or go to an urgent care and have them get you a referral.

Who is "they"?: You might have "growing pains", but these are usually limited to one leg at a time.  There are several more worrisome things that could cause several joints to hurt, so you should definitely ask your parents to take you to a doctor to be tested for arthritis.  Good luck!

Varies: There are multiple potential cause of painful, swollen elbows in a patient with no injury; not likely related directly to your atrial fibrillation or bad knees.  Most common causes are usually arthritic or inflammatory such as gout or an ilex rankin bursitis for example.  Don't hesitate to be seen for an accurate diagnosis and discussion of treatment options.

Spine specialist: In my opinion Grade one retrolisthesis is not a thing ,wiltze classification was for anterolisthesis originally i dont recall him  describing grades for retrolisthesis and later "white and panjabi" did not consider this instability in the orthopedic sense of the word . hand weakness deserves a neurologic evaluation hope this helps

Unclear polyarthriti: The RF (Rheumatoid factor) assay -which is, I assume the "RA factor"- varies widely from lab to lab, so I am not sure if 8.6 IU/mL is in the reference range or not (the lab should give both the result and the ref range).
