Yes: in the context of fasting, I'm doubtful that insulin has to be there at all. High levels would imply either a start of type II diabetes or uncontrolled insulin secretion from an insulin secreting tumor (non-cancerous moslty).

Not necessarily.: Pump therapy may help, but it may be possible to optimize your therapy without the pump. If you are at the stage where pump is a consideration you should be seeing a physician with a lot of experience with diabetes before going on. Recommend good internist or endocrinologist or diabetes center.

5 hr gtt: You need a 5 hour glucose tolerance test. Your notation that "glucose is normal" and "post-prandial Insulin is 66" may be mis-timing the changes that occur during your symptoms. These changes are quite dynamic, changing over minutes.

i have pcos. my glucose tolerance test was perfect. my insulin level was so low it couldn't be found. i was told that's not good. why is that bad?

It's not good: Or bad -- just a test that should have never been ordered since the test is meaningless in that situation and most labs can't do accurate Insulin levels to begin with. If you didn't have insulin, you would have lapsed into a coma from high glucose levels long ago. The diagnosis of pcos is not made by a GTT or Insulin levels.

Does not matter: What diet name you pick is irrelevant. You need to reduce your caloric intake, reducing it by half would not be extreme. Also increase your physical activity. Monitor your weight and aim to reduce your weight by one pound/half Kg per week. If you do not meet the goal reduce your food intake even more. It would be useful to consult a nutritionist.

Meaningless: Get your money back from whomever ordered an Insulin level on you in the first place.  They are almost always meaningless, and the routine lab assays are notoriously inaccurate, even when ordered appropriately.
If you truly had a low Insulin level, you would have already been diagnosed with type 1 diabetes.

Insulin: The Insulin level rises in order to store excess carbohydrates that were consumed at your last meal. The body only uses about 35 grams of carbs at one sitting. The rest get stored and Insulin is responsible for making the storage happen.
