Get evaluated: Does he have shortness of breath? What med is he taking? Does he have bubbling of urine?  Leg swelling can come from med side effect, hear disease like chf, kidney dse like proteinuria,  low Albumin from liver dse, etc. He needs to see a dr soon. He needs to be treated promptly. Could be serious: If you have no history of swelling (edema) and especially if you have new onset chest pain you need to go to the ER right now or call 911.  These symptoms could be due to heart disease. There are other causes, but it is crucial for you to be certain that that is not the reason, and risk serious consequences by delaying treatment.

Serious Matter: Leg swelling, aka edema can be evidence of a more serious underlying issue.  Blood clots and heart failure are just a couple to mention.  You need to get seen by a doctor ASAP to get this worked up and find out the cause.  Don't delay, see you local doctor today!

Hard to say: As this most commonly would be related to vascular issues, possibly arterial or venous or both.  But his best bet is first see a doctor to better identify the cause, as it very well may not be related to anything previously mentioned, and to be able to better guide you/him, the actually cause of his symptoms are very important. Yes: A random blood sugar of 200 or more diagnoses diabetes. A 177 is at the very least pre-diabetes very close to full diagnosis. We know that there can be the known consequences [changes in the body] of diabetes even before diagnosis criteria satisfied. First and foremost  is the be evaluated medically.

Depends ...: First have to know what's causing your legs to swell like that. The cause could be cardiac, liver, kidney. You need a good workup by a good internist who will get a thorough history, a complete physical exam, a laboratory exam, maybe an EKG and CXR and when all the data is in have a long heart-to-heart with you on what's been found and why and what can and should be done about it. No blinders!

Take better care of: At your young age(22 year old) you should have better health that what you have described.
To achieve good health you have to take better care of your body and mind. This includes adequate sleep(7-8 hours daily), healthy Diet(low fat, plant based diet, free from junk fast foods and red meat) and some time(at least 30 mins) reserved for Exercise which can be a brisk walk in the morning or evenings

Tests: You didn't mention what the test was.  This sounds like hemosiderin deposits from blood extravasating from vericose veins - but I am speculating.   See your vascualr doc or get a referral from the family doc.
