Neither: In the world of 3rd-party medical payers (managed care & insurance companies) payment is more for procedures than for discussing things with patients, period.  Consider surgeries, stent placements, radiological procedures, diagnostic tests like colonoscopies, etc.  Comparatively insurance companies don't value the time physicians spend counseling you about medicines & their alternatives.

Yes and no: Generally physicians considered experts in their field are paid for lectures, research, etc. 
This does not necessarily label then as "company people" since their lectures are supposed to be devoid of bias.

It depends: It totally depends upon how far in one's training you are. Intern, resident, attending, etc. In private practice it's really all about your business skills. You should be able to find national averages by specialty online. Best of luck!

Wait for insurance: Don't pay before insurance completes their transaction.  By contract, the physician or hospital must accept the insurance maximum for that service. This means the overall charge may be reduced.  Wait for the insurance to pay the doctor first or call the insurance company to verify your obligation. After insurance pays, you will get a revised bill.  If you are due a refund, demand it - it's the law.

Tough problem: Men consider it "manly" to put up with discomfort and illness.  A visit to the doc implies some loss of that manliness.  You should find out about his condition (use the internet) and discuss with him the consequences of having it taken care of versus the possibility of chronic problems and disability.  The cost is a difficult one but many docs will negotiate if you offer cash.

i already paid for my consult and the doctor did not put in my prescription at my pharmacy. how do i get this issue fixed without being charged again?

Call them: Sometimes people forget.Also sometimes with electronic prescriptions it goes to the wrong pharmacy or a glitch in the system where it is ordered but not transmitted.Therefore if the doctor was to send in prescriptions call them and they can resend it.j

In country: You will need to do a video consult in your country in order to receive the benefits you described. Call your original dr's office and ask where this is available locally. They usually have some kind of referral list available.
