Neither: In the world of 3rd-party medical payers (managed care & insurance companies) payment is more for procedures than for discussing things with patients, period. Consider surgeries, stent placements, radiological procedures, diagnostic tests like colonoscopies, etc. Comparatively insurance companies don't value the time physicians spend counseling you about medicines & their alternatives.
