We would need a complete medical history and medications you are on; habit history(including tobacco usage); and a complete exam. 
One item that varies in size can be a 'geographic' tongue.
Based on above is the only way be can tell if it is safe to only follow or if you need a biopsy now.

It is so hard to diagnose lesions like this over the internet. Since you have already seen a physician, please go back and have him/her reevaluate the lesion and see why the medication they gave you does not seem to be working.

hello i have a white patch on the back of my tongue. i was wondering if you can help me diffrentiate between oral candida and leukoplakia.

TRY TO "REMOVE" IT: You should  "wipe" the patch with a piece of gauze( or clean clothe). Leukoplakia  (the lesion)  will come out and you will have some bleeding where the  "plaque" was. This could also be due to candida, but this condition would probably be all over the mucosa (numerous plaques).
Keep in mind that any lesion in the back of the tongue could be cancerous. Watch!

I : I would recommend first exam by your general dentist. Usually if the white lesion stays for more than a week e general dentist will refer you to an oral surgeon or oral medicine specialist for a biopsy. You might be biting your tongue and the dentist might smooth the sharp cusps to check if that helps.In any case see you dentist asap first.

without seeing this I have two thoughts. The most common lesion with a white border, swelling and pain may actually be a cold sore, which is usually the herpes simplex.

Other than that it may also be a bite injury but usually the patient will remember the bite.

Leukoplakia : Leukoplakia is a clinical term to describe a white patchy presentation on tissues.  This is not a tissue diagnosis and my represent something as simple and benign as a callous like lesion from trauma to something more ominous like cancer.  The presence or absence of pain is not always a good indicator of the nature of the problem.

I recommend that you scheduled an examination with a board certified oral and maxillofacial surgeon or oral medicine specialist who can examine you and determine if a biopsy (taking a tissue sample for analysis) is warranted.
