Avoid...: As a COPD patient, you need to avoid second hand smoke as much as possible since this smoke will result in further airway inflammation, resulting in more symptoms and faster progression. If you wear oxygen, you absolutely cannot be around anyone smoking, ever, since it can cause a fire! key word: avoid!

Supplemental oxygen: It is very common to prescribe supplemental oxygen to patients with copd. During infectious flare ups of copd, it is quite common that a patient requires higher concentrations of oxygen. High flo is a higher concentration of oxygen. Over time, as a patient clinically improves, the oxygen concentration could be tapered slowly.

Depends on factors: Such as body mass index, degree of airway obstruction,  degree of uncomfortable breathing, exercise tolerance, smoking status, presence of complications including pulmonary hypertension, right sided heart failure, chronic respiratory failure, degree of carbon dioxide retention, frequency of exacerbations, and other medical conditions. I have had patients die in their 50s & some that lived 90+ yrs.

There is no right : Answer. It depends on the severity of the cold, type of surgery and other medical issues. If possible, choices that lead to decreased narcotic use or a non-general technique may be advised.

No minimum: There is no minimum. Ambient air is 21%, if it is not enough, you go up from there. Oxygen delivered by a nasal cannula starts with 0.5 lpm and goes up from there by half liter increments. Common starting rates are 1 - 1.5 lpm.

Medication delivery: Pulmo aide is one brand of nebulizer. There are many others.  Patients with COPD benefit from medications delivered directly to the lung by an inhaler or a nebulizer. A nebulizer is useful for those who cannot use an inhaler effectively.
