Not necessarily: So there are findings on the MRI consistent with patellafemoral instability as well as damage to the undersurface of the patella.  Non operative options include therapy or injections and bracing, surgery may be an option of you fail nonoperative care.

It can be...: Pain after Viscosupplementation injection is not uncommon. Red flags would be pain that is getting worse not better,  significant swelling,  and/ or redness about the knee. Call your doctor if pain does not improve.

Try conservative rx: Seems like most of the damage is under the patella. If you have locking you might need arthroscopy, I would first try terminal extension excercises training the quads without putting much pressure on the patella. In addition research has shown that Asperin could help the surface cartilage heal. In addition, MSM (Puritan Pride) could be helpful as a nutraceutical supplement.

Degenerative proces: What it MEANS as stated and what it MEANS in the context of the clinical situation may be 2 different things. Chondromalacia means degeneration of cartilage at the pinnacle point of the kneecap with swelling beneath the kneecap area.

Loss of cartilage: Chondromalacia means loss and/or damage to the cartilage that covers the bone. In your case, you have damage on the back and inside of the kneecap, and grade IV is the most severe grade of injury/loss. Patella alta means the kneecap is sitting higher (closer to the hip) than it normally should. We treat patients, not necessarily mris.  How you are treated depends on your symptoms and disability.

Several: Resting the knee for a short period of time and taking nonsteroidal anti-inflammatory drugs (nsaids) such as ibuprofen, changing the way you exercise, performing exercises to both strengthen and stretch the quadriceps and hamstring muscles, losing weight, special shoe inserts and support deviceaping to realign the kneecap, wearing correct running or sports shoes, surgery for pain behind the knee.
