Be careful: Reputable clinics should.  Be very careful.  There seems to be a "pain clinic" on every corner.  Some clinics are nothing but pill mills managed by doctors w/ no certification in pain management.  Be sure the doctor is board certified by american board of pain medicine or american board interventional pain practice.

No clinical sx: If no clinical symptoms criteria for disease not met. ANA can be very reliable in diagnosis with symptoms consistent with disease state.  But false positive results occur. The question should be why was test run.

Stop worrying: I am going to assume that your rnp/smith are below the ranges that the lab considers significant. All immunoassays turn up some positivity because the of cross-reactivity of other antibodies that you may develop from meeting different micro-organisms or goodness-knows-how. The miniscule rise in activity means nothing, your physicians were right to say thing ; if you had mctd you'd know u r sick.

symptoms are key: If you don't have any symptoms but have slightly elevated antibodies to rnp and ENA (ribonucleoprotein and extractable nuclear antigen) there isn't any need for treatment. Diagnosis of mixed connective tissue disease, lupus, and similar conditions does require a thorough history and exam - best done by a rheumatologist.

Accepted Answer

It : It has to do with medicare laws.  It is illegal to charge any patient less than you charge medicare for the same service. Thus, if you accept medicare patients, which most doctors do, you cannot give free care in your office. Even free screenings can get you in trouble.

The only solution is to drop medicare form your existing practice--which defeats the purpose of providing care to underserved people--or to establish a separate practice that does not take medicare.  However, that separate practice will have its own overhead costs, and if you don't charge for your services you can't cover your out-of-pocket expenses.

For the record, I do volunteer my services at a university hospital, but that only saves the money for the hospital--they still have to charge the patients or they would violate the law.  And also for the record, though such donations of time are worth a great deal of money, pro bono services are not tax-deductable.

Travel med clinics: Yes, travel medicine clinics are usually staffed by infectious disease specialists, or they may be staffed by a family doc or internist who has a special interest in travel medicine, has done extra training in this area, and has full up-to-date access to all of the latest health information everywhere around the world.

what type of doctor can clear me for a "lapse of consciousness behind the wheel" ? my local clinic won't do it.

Neurology: A Neurologist can do this. However, unless due to a medication you do not need to be on, you should seriously consider the dangers driving could pose to you and your community and consider a new lifestyle in a new place with no driving.

are electronic home bp machines reliable .. i have omron hem 7201 .. it mostly shows less bp then what i see in doctors clinic.

Generally accurate: Not unusual to see lower BP at home.  Next visit bring your unit to doctors office & compare readings.
