i have a pinching pain on side of index finger.  i also wake up in middle of night either with pain; tingling especially with arms are over not down?

Nerve: This is consistent with a nerve impingement, possibly at the wrist causing a carpal tunnel syndrome. When your wrist is relaxed during sleep, the impingement is most pronounced. A hand surgeon or neurologist would be best qualified to evaluate,advise and treat you. Treatment options include wearing a splint, corticosteroid injection at impingement site or surgery.

Carpal tunnel: This is likely carpal tunnel syndrome. I would suggest wearing wrist splints to bed as a first step. Remove them when you arise in the morning.

Need to see a doctor: This could be either neck problem or possible carpal tunnel syndrome. You need to see a doctor to get a completely neurological examination. Did you ever had neck injury during sports or motor vehicle accident in the past? If you did, it is possible the numbness related to the neck injury. If you did not it is possible carpal tunnel. I still recommend you to see a doctor to rule out other causes.

CTS: Carpal tunnel syndrome is the commonest cause. Neuropathy from diabetes or other causes can cause similar symptoms.
