Coumadin (warfarin) for life: I assume that you have been tested for a hypercoagulable state (e.g. Factor v leiden). Given your history, most doctors would place you on warfarin for life to prevent DVT and life-threatening pe.

Ankle pain: Sounds like neuropathic pain which may be related to peripheral neuropathy or superimposed "tarsal tunnel syndrome".  Treatment may involve removing compressive forces from the area (shoes), or controlling movement of ankle(orthotics), and/or the use of medications specific for the pain from nerves.  Also in the same area are the tendons the go to your medial foot, and toes, can be tenosynovitis.

i had a sigmoid colectomy 9/2018. 3 weeks home i developed massive portal vein clot. i have had ab pain, nausea, excessive weight gain since, tho i dont eat. labs and cts remain good. what was missed? i feel awful and cant work. my health tanked!

High index of susp.:    PVT is an uncommon complication following laparoscopic bariatric surgery, Gen. Surg. with potentially life-threatening consequences. Presenting symptoms are often vague, and a high index of suspicion is required to obtain the proper imaging studies to confirm the diagnosis .  Approximately 2 wk  post operatively, Rapid wt Gain,  nausea, Vom,,  abd. Pain . Labs - High WBC and mild LFT at times.

Usually will shrink up or atrophy.
On occasion will recanalize.
In the clinical situation of varicose veins and some clotted branches " stripping" can be a useful tool.

Talk to your Dr.: The Basilic Vein travels both in superficial and deep tissues. The specific location of the blood clot will determine whether is is a deep vein thrombosis or a superficial vein thrombosis. DVT is an emergent situation and will be treated immediately. SVT is not emergent and can be treated a month later or more. Talk to your Dr for specifics on your imaging results or other diagnostic tests.

If treated: If your new dvt is being treated with blood thinners running and exercise is allowed after treatment begins for a few days. If the clot is older there is no reason you can't participate in running and exercise. While you're on blood thinners be careful not to participate in contact sports.

Compression: Some type of process compressing the vein would cause a occlusion. This could be a a mass or some other structure in the neck occluding the vein.

Veins vs. clots: Blue worms that are not painful are most likely to be varicose veins.  Pain and swelling, with or without varicose veins, is more liekly to be a blood clot.  Regardless, if you have varicose veins that are painful, you should definitely get it checked out by a vein specialist.
