Pain: Scar tissue formation is very common after surgery.  Sometimes, scar tissue (adhesions) will cause pain and other times will not.  If you have no pain, then there is no need to do anything.  However, scar tissue is less likely to form after vaginal hysterectomy than abdominal hysterectomy.  Please see your doctor if you are having any issues.

Not necessarily: Because this part of the body has a very good blood supply, injuries or tears can often heal with barely a noticeable scar. That is the reason labiaplasties and vaginoplasties are becoming so much more popular. The function and appearance can both be improved with surgery with virtually no scarring.

Heating pad +: A heating pad can assist with your discomfort, but it is a little early to ascribe the pain to "scar tissue"&gt; Did your surgeon exam you and provide testing to determine causation? If not, a revisit to your MD would be helpful for the same.

Adhesions: Any intra-abdominal process like a hysterectomy can cause scarring/adhesions within the abdominal cavity. It does not cause actual scarring of the colon unless it was injured intra-op.

Yes possible: Yes it is posssible. An ultrasound is simply a shadow like image and it does not necessarily tell you the composition of the structure. Hysteroscopy with possible biopsy may be useful in differentiating between the two.

i had a terrible rash that started from my bikini line, some days ago. it spread close to my anus. it left a dark scar. i had no vaginal itching. help?

Possibly: A scar neuroma may be capable of sending abonnormal pain signals. You will know if this is causing it is if you press down on you scar and it reproduces your symptoms. The best way is have your doctor examine you.
