hi docs, i have rotten teeth and i have burning pain almost everywhere on my body. could my teeth be the cause? the pain is so bad i contemplate suicide. i'm on oxycodone and methadone. what tests should i get? what could be the cause of my pain? ty?

: Decayed teeth need to be evaluated and treated by a dentist. Periodontal issues may also be present. The infections in your mouth may be traveling by your bloodstream throughout your body. See a dentist ASAP for proper case.

what are side effects of methadone and oxycodone?

Many: I personally do not use Methadone as i feel the dangers outweigh any advantage over other opioids.  The most significant side effects are that of breathing suppression and heart electrical conduction problems.  Oxycodone has similar side effects but is much safer.

if a person on methadone for chronic pain is also prescribed prn oxycodone,  how long should they wait after taking the methadone to take the oxy?

Breakthru : methadone is a powerful analgesic with a long half life. Oxycodone is a short acting powerful pain medicine to be taken in your case only during a breakthru pain crisis and certainly not  by the clock. Both medicines can cause side effects and even death.  I prescribe this regimen for intractable chronic pain only and monitor for its helpfulness and safety

There is no answer: Concerning the pharmacologic treatment of pain, there really is not "better than" or "as good as". Both drugs are opioids and work by "binding" to opioid receptors in the brain.  I do not personally use Methadone as it has a very high potential for adverse problems thus a low safety profile. Some opioid receptor binders are more affective than others in some individuals.

Neither: Long term use of narcotics carries considerable risk. Of the two, methadone carries the greater risk.

i have been changed from oxycodone 180mg to methadone 50mg for chronic pain (neuropathic) is methadone being prescribed more for pain these days? why?

Accepted Answer

Less psychological: methadone is an inexpensive pain medication but is very effective and hits NMDA receptors and helps more with bone pain and neuropathic pain  It does not have the psychological high so it is not psychologically dependent like other opioid  It is longer acting so you need to be very careful taking it and taking more than prescribed  It is definitely the best long term chronic pain medication

can i use oxycodone to help the withdrawal from 17yrs of methadone use daily,  my goal is to be off methadone, i have tried suboxone but relapsed.

Bad idea: Hard to get good help though if you have been getting it on the street.   I suggest you keep at the suboxone.   It is your best bet outside of a methadone clinic or seeing a specialist in addiction medicine.

Opiods for pain: Hydromorphone 8 mg is roughly equivalent to Oxycodone 30 mg.  I assume you are asking re oral medications.  Both of these are effective for moderate to severe pain.  In the US, Methadone is restricted to programs for treating opiod addiction, and is generally not used for pain relief.

Accepted Answer

All different: They have different uses and modes on administration. Hydrocodone and oxycodone are taken by mouth and last about 3-4 hours. Methadone takes several hours to days for its full effect and last a very long tie. Fentanyl is available through a patch or IV and is short acting and extremely potent.

is it unusual for a doctor to prescribe oxycodone along with methadone?

It would be for me: But-I'm sure there are situations and justifications for that sort of combination when dealing with pain management situations of special gravity or in patients who may have terminal cancer illnesses who are in completely unremitting and relentless pain.  But in general, I would think that either 1 or the other in high enough dose or frequency of dosing should be sufficient for most types of pain.
