all tests came back negative for yeast, bacteria, trich, and stds but vagina still itches pretty badly. 7 days of diflucan didn't help. what can it be?

Simplify: You could have cytolytic vaginosis which is a condition where the normal bacterial flora in the vagina are shifted. Things like douching or soaps can lead to this. Watching with warm water only, wearing cotton underwear, avoiding tight pants and sitting with your legs crossed for a long periods of time can help.

Usually not: A urine analysis, especially in women, can be misleading regarding the presence or absence of a uti. Pmns, leukocytes, white cells (all the same) and some bacteria are quite common findings in a urine sample and are usually just contaminants from the vagina especially if unassociated with any symptoms of a uti. A urine culture is the definitive test to rule in or rule out a uti.

Trichomonas ?: The problem with Trichomonas is after treatment, it may disappear, but if you or your partner(s) have "other partners", transmission is inevitable, and the odor will seem to not go away.  Get another test for Trichomonas, and ask any other contacts to get tested as well.  Not everyone will "confess" to having other partners , so it may again return due to re-infection.

Examine your soap: You need to check and make sure that your underwear is fully rinsed if machine washed as the material can retain dried soap powder or liquid which will reactivate when confronted by a moist vulva. Secondly, make sure that you use only Dove, Ivory or Caress soaps on the vulva as other soaps are too harsh and can cause itching.

how does the std trichomoniasis originate?? i understand it is a parasite and is passed from penis to vagina, vagina to vagina, etc, but how does it begin? from an unclean penis/vagina that wasn't washed properly?? what invites/feeds the parasite??

: Trichomoniasis doesn't "begin" or "originate" anywhere. Every case is caught from someone who already has it; the trich parasite evolved in humans and probably for thousands of years has been transmitted from person to person, primarily by vaginal intercourse. Hygiene or cleanliness has nothing to do with it. It feeds on chemical nutrients that occur naturally in the vagina and urinary tract.

Probably not: There has never been research to tell for sure, but probably there is no risk for trichomonas without actual penetration.

Recheck: You should see your gyn to make sure your infection has cleared.  Also-- if your partner was not also treated, you might have been reinfected and may need to be treated again.

can trich get into the blood stream (if you have been infected) through a scratch in the vagina?  if so what happens?

Not really: Trichomonas vaginalis only stays in the genital tract, and doesn't cause disease elsewhere.  In the literature, if the patient is profoundly immunosuppressed with medication or cancer, there have been some reports of other trichomonads causing systemic infection, but not vaginalis as far as i see.  If your immune system is ok, you will be fine with standard therapy, scratch or no.
