Elevated creatinine: Your has to be seen by a neprhologist (n) for a diagnosis of his renal problem. His GFR is estimated, not actual. He needs to be examined, have a 24 hour urine collection, to get his true gfr, and protein. Labs are needed to see if he has diabetes or another disease affecting his kidneys. A renal u/s also need to be done. Once the results are back, a prognosis and treatment plan can be offered.

Yes do treat your BP: You have stage 2 chronic kidney disease with proteinuria and also elevated systolic  BP indicating that you are at risk of this getting worse. Thankfully you're sugar level is low so not a diabetic - the second most common cause of kidney disease.  Ask your doctor about a low dose ace inhibitor or an arb medication for bp.  A good alternative to these if not tolerated is CCB (amlodipine). Overwt?
