Antiplatelet meds: First, why is artery occluded?  Could be due to local thrombosis, but could also be due to traumatic or spontaneous dissection.  Secondly, are there risks for stoke such as diabetes, hypertension, high cholesterol/triglycerides, high homocysteine?  Talk with your doctor re prevention, perhaps clopidogrel, aggrenox.  Many areas need to be addressed.

None: Most coronary fistulas to the mpa are tiny, and if there there is no obvious shunt it will likely not require rx or cause any problems such as bradycardia.  Check with your cardiologist for f/u.

Yes : For the noise in your ear they make noise damper devices. There is no medication in surgery would do a lot more harm than good as long as the blood flows normal in your  Carotid artery. As far as mitral valve prolapse and you do have to have more information from your doctor cardiologist regarding treatment in most   prolapse and without Regurgitation mitral valve does not require any treatment

Lower than what?: Lower cost than what? Pentoxiflyline 400 mg 3X a day as a generic meds cost $120 a month and is indicated for claudication, not ED. I'm not aware that either pentoxifyline or shock wave treatment is effective. How was the diagnosis made? If you had an arteriogram, I suspect angioplasty is your best bet, but at your age, 37, I suspect either you have small vessel disease or the dx is wrong.

Medications enables the heart to deal with blocked arteries but generally does not remove the blockages.  Angioplasty is able to remove the blockages so that blood flow is improved.  Surgery provides an entirely new source of blood flow to the heart.
