Not that likely: But maybe. A hernia can allow internal organs to come out and into the hernia sac. In women, the fallopian tube and ovary can get into the sac at times. Tugging may occur and cause discomfort, but the exact symptom would be hard to predict. A ct scan or us might help define if this were happening. Other organs that might get pulled into a hernia include bladder and bowel.

ANS: your hiatal hernia is not your gall bladder so not certain what you are asking. Ultrasound is not 100 % diagnostic nor is a surgeon's history and physical exam. But my guess he probably is more likely correct than the ultrasound.

Quite accurate: In proper hands of skilled ultrasonographer, detection of epididymitis and inguinal hernia can usually be diagnosed. Other problems such as testicular torsion, hydrocele, varicocele, and undescended testes  are also identified.

Mixed: A hernia can be detected relatively accurately with an ultrasound and even better yet with physical exam.  
Epididymitis is slightly more difficult.  It can sometimes been seen on ultrasound, but it is more of a clinical diagnosis and is treated with NSAID (anti-inflammatories) and sometimes steroids and antibiotis

Abdominal/ventral He: Diagnosed by physical examination and scans.

More info: Each imaging modality can be optimized to help learn more about diseases and help answer questions your for your doctor and you.  For example, ultrasound is good at looking at the gallbladder; ct can show bleeding in the brain; MRI can show an acute stroke or find masses in the liver.
