Might need: Antibiotics . You did not say how long ago you stepped on it. If it just happened you could try soaking in domeboro  three times a day for 15 minutes but if there  is a chance of leaving a splinter behind or if it has already been a couple of days the you should probably seek care.

i have about half of a toothpick stuck inside the bottom of my foot, i sanitized it with alcohol , however the toothpick is so thick in there , its just a hole, and no toothpick is sticking out enough to grab it. what do i do?

: Go to your local urgent care center, a local surgeons office or to the emergency center.  If you do have a Foreign body embedded it will get infected, thus it should be removed.  Sometimes with very small foreign bodies, its easier to let an infection start so that the F.B. can be more easily localized.  In any case this type of injury needs follow up by a health care professional.

Unrelated: I would suspect your symptoms are unrelated or stress induced because you are waiting for something bad to happen. I can't put the symptoms together with anything such an experiment would do. Take a hot bath or sauna and relax. It should pass. If problems persist see your doc.

Yes but don't do it!: An unpopped blister represents a closed sterile environment.  Yes on occasion one can become infected then it is time to open it, however the risk of infection in a dressed open wound is still higher than that of an intact blister.  The blister will gradually resolve with time.

Salt Water: You could try soaking the foot in a bowl of warm salt water. You can make the salt water yourself, adding enough salt to make it salty but still dissolved. The salt content of the water will gently draw the foreign body out of the foot. Understand that this may not happen right away but should cause movement in the right direction. For pain relief you can try motrin or naprosyn (naproxen) as directed.

Toothpicks: Swallowing a toothpick can become serious. The smaller the piece, the less likely you are to have a problem. There is always a chance of a perforation however, in the stomach or colon. Your primary care doctor can help monitor for dangerous symptoms in this situation.

: Don't waste time or worry on something that will pass out of your system within 3-4 days without any noticeable effect. We have plenty of experience with foreign material ingested by kids, adults have even less trouble. Eating stewed okra or similar vegetative material will form a congeal around any foreign object &amp; help it pass through.
