PCP for P/E/: Seeing blood in the toilet, on the outside of your stool, or with wiping  is common. Fortunately, most of the causes of such rectal bleeding are not life-threatening; In addition, bleeding from higher in the digestive tract, such as the stomach, can produce black, tarry bowel movements because stomach acid turns blood black. Bismuth (such as in Pepto Bismol) and iron supplement does same.See Pcp

BRBPR: This may just be hemorrhoids from straining but bleeding from rectum (brbpr) may also indicate anal fissures, proctitis (due to infection, inflammatory bowel disease, radiation therapy, stercoral ulcers, rectal prolapse, trauma, etc.) or a bleeding source further upstream. Depending on your age, risk factors, current medication (aspirin, nsaids, anticoagulants can cause bleeding), see your doctor.

i have been straining to defecate & having incomplete evacuation since 2 weeks. i noticed small drops of blood on toilet paper, on the surface of stool & some drops mixed with mucus that falls out of my anus a few times. is this serious?

Common rectal issue.: The lower GI system produces mucus.  You can pass some blood with staining.  You need to look at your diet and possible allergies.  
Your symptoms can be serious, but a good exam with your doctor will be helpful.

Maybe hemorrhoids...: ...but you can't assume it. That's probably the most common cause, but other things are possible. Mostly not serious, but don't take the chance; see your doctor.

Anal fissure/piles: Your symptoms are consistent with anal fissure and/or piles. You should see a doctor as blood loss can lead to anemia. To keep you stool soft,  take a fiber laxative like Metamucil, drink enough water daily so that your urine is colorless. during bowel movement today, i had very hard stool and then had blood on toilet paper (not just one tiny spot like other times). i have psoriasis on anus area. and i think hard stool could cut my skin either eternal or external.should i worry?

Old vs. new issues: Blood in the stool, trouble pooping, a lump at the anus, or anal rashes are symptoms that need to be evaluated by a primary care doctor or a gastrointestinal specialist. The cause can be a hemorrhoid, an anal fissure, an infection, etc... Cancer in that area would be ultra rare in a teen. If the cause is already known by the doctor, a teen can just follow her doctor's care instructions.

Not nl: Many things can cause this. See your doctor so he can exam you and consider lower GI work up. Anoscopy, proctosig and or colonoscopy.

Exam!: Your symptoms lead to various diagnostic considerations including a lesion, ulcer or even cancer. You therefore should be examined as soon as possible by your Family Physician to determine the proper diagnosis &amp; treatment. And make certain you are well hydrated with healthy fluids such as water or juice &amp; avoid alcohol, smoking &amp; illicit drugs. And include adequate fiber in your diet.

No: This is common after a difficult / hard stool. The "pop" wisdom that is represents hemorrhoids has less to it than that you might simply benefit from perhaps more dietary fiber and/or drinking more water.

i saw multiple small red bits in my stool and when i pressed them with toilet paper, they spread/dissolved leaving a bright red streak on the toilet paper. could it be tomato flakes or is it blood in my stool?

: Tomato skin would not dissolve. You can buy a kit from your pharmacy to test your stool for blood. It would be better to see your doctor for an examination as even simple hemorrhoids can enough blood loss to result in anemia.
