The safest thing: To do with pathologically proven benign nodules that demonstrate that kind of interval growth is repeat fna biopsy. The procedure has a very low complication rate and most patients find it to be painless. Other measures might include TSH measurement and, if it is depressed, a radionuclide scan to see if the nodule(s) are "hot" or producing excessive thyroid hormone. These nodules are benign.

i had my thyroid ultrasound done and i have a goiter and some nodules. what do i need to do now?

Thyroid function?: You need to know your thyroid function level (free t4, tsh).  If these are normal, the nodules are small (&lt;1 cm), and you have no neck compressive symptoms from the goiter, usually observation is recommended.  If a nodule is &gt; 1cm and/or has suspicious characteristics on ultrasound, a fine needle biopsy may be needed.

Possibly thyroiditis: Hypervascularity is a typical finding in people with underlying autoimmune thyroiditis (hashimoto's or graves disease).  The 3 mm nodule is likely of no significance and can be watched.  You need to know your TSH level.  Getting thyroid antibodies (thyroid peroxidase, thyroglobulin antibodies) would be helpful in making a diagnosis.

: Your thyroid is currently hypoactive and is trying to make more hormone which causes increase in size. Your synthroid dose needs to be increased in order to bring your thyroid into proper range.

Multinodular thyroid: The most common cause of your ultrasound finding is a multinodular goiter. Majority of time these are benign and the thyroid will have normal function. It is actually more of a concern for cancer when the thyroid only has 1 or 2 solid nodules, not dozen of smaller ones. A thyroid biopsy is a simple test to sample suspicious nodules.
