Sedation: Your problem of anxiety is very common.  Tell your dentist and ask if he can help with something to help with your anxiety for the visit or sedation.  If your dentist cannot help,  ask for a recommendation to a dentist who does use sedation.Speak to your dentist and ask for a prescription for a mild sedative.

Been exposed?: Some vaccines are available, and meds can be used if you acquire meningitis.  At this time of year, west nile virus, and a series of equine encephalitis problems might arise.  Do not stay quiet, contact your doctor and discuss this fully, including your fears.  If too overblown, some counseling will help.

Pregnancy: Many women are concerned about body changes and pain that can be associated with pregnancy and delivery. The best way for her to counteract her fears would be for her to become more educated about it.

No pain before: I do not know what surgery that you will be having, but there would not be expected to be pain before the surgery. There may be very minor discomfort with inserting the small intravenous catheter, but well trained nurses make the sensation minimal. Otherwise there would not be pain before going under anesthesia.

i  occasionally smoked cigarettes and hookah for a year at 15, did i  permanently cause damage? i  am terrified.

Permanent? No!: Some? Yes. But the longer you stay away from cigarettes, cigars, hookah, 2nd hand smoke, other illicit drugs etc, the better off you'll be. Some studies suggest that quitting at 80yo can be helpful (although clearly not as helpful as if quit at 15yo). Best is to never smoke at all but we can't go backwards in time. But since that was 5yrs ago, time to stop worrying ; being terrified. Time to live.
