What symptoms?: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, your symptoms are unlikely to be due to your hemoglobin/hematocrit.  It would require investigation to find the cause of your symptoms, which you did not describe.

Barely high: Overall the Hct of 47.7 is not much higher than the upper limit of normal, about 45.5 for an adult man. If you smoke, stop. Do you have sleep apnea? Smoking and sleep apnea are two common causes that can raise it.  The MCV though is a different story, and is reflective of altered production of red blood cells in the bone marrow. I recommend you see a hematologist to sort this out. Be well.

If you were my: patient, I would treat you for B12 deficiency after also checking MMA (methylmalonic acid) and homocysteine. 200 pg/ml is usually considered the lower limit of normal but with modest anemia, elevated MCH, symptoms and family history I would go ahead and treat you. : You have polycythemia or increased red blood cells that can be seen in many conditions aside from blood disorders. Your main doctor should be your main resource, a hematologist’s opinion may be needed.

Polycythemia: A hemoglobin of 18.3 is just below the acceptable normal range of 18.5 in a male (some sources consider 18 to be cutoff).  Any condition that causes an increase in red blood cell production by the bone marrow will cause an elevated hematocrit.  A serious form of this is called poylcythemia vera, other conditions include living in high altitudes, tumors, heart&amp;lung problems, etc.

W/u & phlebotomy: You should go to see a hematologist to get more work-up to find out the cause of your polycythemia and you definitely will need to get phlebotomy now. If this is a bone marrow problem- phlebotomy is the treatment. If this is caused by other conditions- like lung, kidney/liver problems, drugs etc- the treatment is to fix the underlying condition. Go to see a hematologist-or er if you have symptoms.
