Pain: There are several routes you can take on your own.  Call a local hospital and ask for a referral , call the local medical society and see if there is a referral service.  If that fails , call an orthopedist and ask if they do that or can make a referral.

Back surgery: You appear to be very highly motivated and cognizant of your medication use. I would probably stay with a Physiatrist or an Anesthesiologist who does Pain Medicine

I do not think so: D5 id your mother walked before the hip fracture? It was 5 years ago and if she has alzheimer's disease she forgot how to walk. She would have hard time in the rehabilitation program.

T1-2 herniation: Consider the  local university  medical  center neurosurgery department. The first issue is how severe is the herniation and if related symptoms . Not all herniation are significant enough to warrant surgery.  You can find board certified neurosurgeons at
