covid vaccine (pfizer). i'm hetero factor v leiden. 1 shot complete and overdue for 2nd. seeing many people get clots/stroke/pe after 2nd. do you think it's better to get 2nd or not? any doctors seeing clots etc develop after 2nd jab?

: It is wrong that "many people" get clots etc after the second COVID-19 vaccine dose. More than 99% of Leiden V vaccine recipients have no problem at all. Without vaccination, the risk of a bad outcome from COVID is FAR higher than the chance of a clotting problem after the vaccine. Just do it! (Personal comment:  my daughter is Leiden V, has had DVTs and PE. I made sure she is fully vaccinated.)

is cancer my next battle? after stroke,  dvt, p.e. i have prothrombin gene mutation g20210a. i'm currently cancer free but what are my odds?

Why do you ask?: You have given me no reason to believe that you have cancer. The cancer that's most famous for causing thrombi -- common pancreatic cancer -- would be very rare at you age of 29. I suspect you have some additional unknown cause of hypercoagulable blood and need to stay under the care of a good hematologist. If you do develop a mass or new symptoms, get seen, but don't let fear ruin your life now.

Stroke: Stroke is most common depending on your risk factors. Pe is very rare.

i have pe but not ed. i get excited very quickly & frequently; stay hard for long if not touched down. after rubbing couple of times/strokes during sex, i ejaculate. what is reason & solution? sometime i masturbate before sex to control urge. no ed?

Accepted Answer

: If you are otherwise healthy then try using a local anesthetic gel on the head of the penis before sex. You also need an understanding partner to calm your anxiety. PE is relatively common and treatable but requires patience and understanding by both partners. If that does not work see a sex thrapist.

the installation of an ivc filter is all that is needed to avoid most heart attacks, strokes and pe's if there is a history of familial heart disease?

No way;can only trap: large clots from leg/pelvic veins from proceeding to right heart&amp;lungs. Heart attacks=clots in arteries supplying heart muscle after plaque rupture. Strokes=clots in brain arteries or from left atria (rarely ventricle); Most=without symptoms; occur without warning. Setup=atherosclerosis (drivers=LDL-P, HbA1c &amp; Systolic BP not ↓↓, Large-HDL-P not ↑↑, etc as in my other answers) &amp;/or atrial fib.

had 2 uedvts. on warfarin for life now. what are the chances of getting another clot while on anticoagulants? i'm scared of stroke, pe, heart attack..

Low: The veins of your arms and legs do not connect either to your heart or brain: the chance of a heart attack or stroke is zero. Pe is a tiny risk. You should report any instance of unexplained (ie not exertionally related) shortness of breath promptly. The risk of recurrent DVT on Coumadin (warfarin) is very low but you must religiously get your inr checked when advised.

Excellent Question: Statins- and other drugs and nutritional supplements that lower your LDL particle number and favorably alter your LDL pattern as much as possible.  Omega 3 supplements to help lower triglycerides and change LDL pattern when appropriate.  Anti- inflammatory diet- avoid sugar, high fructose corn syrup and the like- speak with a nutritionist.  There are other suggestions but this is a good start!

why do some doctors order and consider fibrinogen as a risk factor for heart attack and stroke and not d- dimer? which of these 2 tests is a better risk factor in predicting future (5 months or more down the road)heart attacks, strokes, dvt and, pe?

Accepted Answer

: In creased fibrinogen levels can increase formation of blood clots, which can "predict" increased frequency of cardiac disease.  A "D-dimer" test is an acute phase test to assist in "ruling out" present symptoms of pulmonary emboli, and hence is not a "predictor" of future events.

i'm on anticoagulants,  i've suffered a stroke, dvt, and pe. my wisdom tooth is in severe pain meaning my jaw and left ear should i go to an er?

ER?: Go to the er for any medical reasons or if it is after-hours and you are unable to see a dentist for temporary pain medication (and an antibiotic if necessary). The er cannot treat your dental problem. Please see an oral surgeon asap who can better address the issue with your wisdom tooth.
