ER?: Go to the er for any medical reasons or if it is after-hours and you are unable to see a dentist for temporary pain medication (and an antibiotic if necessary). The er cannot treat your dental problem. Please see an oral surgeon asap who can better address the issue with your wisdom tooth.

Go to see MD: You should seek medical attention so you can get help- either by going to see your pcp -if you can afford to wait until tomorrow morning. If the pain is so unbearable and you do not have any pain medication to help- then you can go to urgent care or er etc- whichever you prefer. Urgent care probably is better as usually you do not have to wait for too long.

Oral surgeon: With your history i recommend an oral surgeon see you if at all possible in case you need more than pain medications or antibiotics. If you take antibiotics be sure to get your inr checked within a week as antibiotics often change your response to coumadin (warfarin).
