Stool: That is not a question with any one answer. It depends on the food you eat, any conditions of the GI tract. And then vitamins and medications can affect this. What is the real question here? Please be specific.

for the last three weeks now my stool texture and color has changed . stool seems to not be well formed and soft and dark  ?

Need to see a doctor: Soft stools which are dark (assuming by dark you mean more on the black side than brown), can be sue to bleeding in the upper intestinal track (stomach).  Change in diet and other conditions can do this. Be sure to see your doctor.

Possibly melena?: Dark-colored stools may result from iron supplements, foods you eat (like a lot of spinach), substances (like peptobismol), but most significantly suggests bleeding. When blood is denatured by acid in your stomach, as from ulcers, stools will turn tarry ("melena"). Blood may also be ingested from nose bleeds, bleeding gums... Persistent melena (without obvious cause) ; symptoms warrants md visit.

my baby is a week old and she is bottle fed (on s26 gold) but her stool is like little clay balls. the texture is also like clay. is this normal?

Balls are not normal: Newborn stools, after clearing out the sticky meconium stuff, are not balls. If balls, then the baby's pediatrician can examine both the stools and the baby. That means taking some good cellphone pictures of the stool balls to show the doctor. Newborn stools are watery or seedy or like sauce. They are any shade of yellow or green. They should not be whitish nor claylike. Breastfeed if possible.

white clear string in stool. is it worms or something more serious? i noticed it when i wiped & it appeared clear & sof rubbery texture. please help.

Unlikely: Pinworms are threadlike but only an inch or so in length. Ascaris are ling thick and similar to nightcrawlers in appearance.The usual origin of "clear stringy" material in stool are breakdown products of vegetative matter, like celery.

basically my stools have come out smaller and feel like there should be more. when they're soft, they're like the texture of refried beans. i'm going more frequently. basically they're not as bulky. should i be worried? help?

: There are many questions that need to be answered concerning this issue: what is the quality of the diet? Any new medications or supplements? Any associated abdominal pain, blood in stool, etc. So basically you need a visit with a doctor in person on a no urgent basis to discuss this more.

Bentyl (dicyclomine): Bentyl(dicyclomine) does not change the appearance of stool. It may cause constipation which in turn may cause more firm stools and increased problems with expelling stool.

Viral Illness: Viral respiratory infections can be associated with stooling changes including change in consistency/color. Maintain regular diet especially encouraging increased low-sugar fluid fluid intake with diarrhea. Avoid otc cold meds and tx cough w/ humidify air, elevate head of bed, or even pasteurized honey if > 1 to - to dr. If rapid breathing, wheezing, high fever, or wet diaper > 8 hr apart.

IBS: You may have irritable bowel syndrome if your change in stools is associated with cramping, bloating and frequent stools among other symptoms.  A gastroenterologist can diagnose and offer treatment options.
