when doing pelvic floor exercises i sometimes get sharp pain in back or sciatica pain running down leg? i have no spine problems.

Sciatic nerve: The sciatic nerve comes out of an area of the pelvic floor muscles.  If you are contracting these muscles or if they are inflamed / irritated the sciatic nerve can be irritated.  Stretching and chiropractic would be helpful.  Also, there are Physical Therapists specifically trained to improve pelvic floor tone.

That is assuming you: Are taking equivalent doses of these medications which you may not be doing.  Also, people can have varying responses from one another when it comes to medication as well. This can also be seen in terms of side effects and tolerances.Also, vicdin is a combinaion of two pain killers, hydrocodone & tylenol (acetaminophen)  while Dilaudid is only a narcotic component.

CONDITIONS LIKE: Multiple myeloma can present with just localized back pain. 
Please review the possibilities with her doctor, particularly when there is a quick, drastic decline in health, in an otherwise healthy, active woman.
Please know, myeloma is not the only possibility.

how bad is a minus 5.2 t-score for spine? it was + 3.2  3-yrs ago. i had 2 steroid inj's in sciatica a yr apart. might this have caused bone loss?

Osteoporosis: You have osteoporosis. It is a very significant bone loss that you have here - from +3.2 and now down to -5.2.
You will need to see your doctor to discuss about this bone loss and also to  take medication like bisphosphonates, calcium and vit d. Discuss further with your doctor. Steroids can cause bone loss- but i will not expect 2 steroid injection a year apart -will give this much of drop.
