Sounds like hives: Rash that comes and goes so quickly is most likely urticaria or hives. Is it itchy? Hives are usually itchy. The rash may develops without any provocation. Sometimes stress or emotional change triggers hives. If it is very bothersome, you should take her to a doctor and have him(her) prescribe medicine. If it accompanies wheezing, swelling of lips, tongue, or breathing problem, take her to er.

This : This is a bit tricky to answer without a picture and more clinical information.  I doubt it was from the blood draw.  This time of year it could be many things including a medication reaction, eczema, a viral exanthem, psoriasis, dry skin from low humidity, and many others.  Perhaps a visit to a dermatologist would be helpful for diagnosis and treatment options.  I hope that helps a little!

Need to examine: It is not feasible to provide a meaningful opinion without examining you, however, you may try elevating your feet and legs at night to help in draining the fluid, if it is due to fluid under the skin. Exercise may help too.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

Can take a while: Depending on how severe the reaction blisters associated with poison ivy can take several days or longer to resolve. The resin on the leaves can really set your skin on fire. Generally one should keep blisters intact but they always seem to rupture. Treating with a potent topical steroid can also help.

Define "kick in": You can usually expect a reduction in the intensity of itching within 24 hrs. Swelling may take a bit longer. The skin rash will take a week or two to fade whether you have the shot or not. There are products like IVYDRY or Benadryl cream that seem to help some.I avoid calamine lotion as too drying and stinky.

Poison ivy: The rhus antigen found on the surface of the poison ivy leaf in susceptible individuals causes a contact dermatitis. The rash only occurs where there is sufficient skin contact. The rash does not spread to un-contacted skin, but may occur more quickly in thinner skin or areas of larger amounts of contact. Contact dermatitis rash lasts 10-14 days. Your boyfriend could indeed have poison ivy.

Eczema: Eczema, in the new, or acute stage, can look a lot like an itchy, weepy poison ivy rash.  It is usually limited to a localized area, rather than be spread over a larger area like poison ivy.  See you doctor to be sure and to get treatment.

my son is 1 and has a small rash in his knee it kinda looks like poison ivy but i know it's not it's about the size of a 50 cent piece.. any ideas?

Dermatitis?: It could be a contact or irritant dermatitis.  Other options include poison ivy- but in 1 isolated location, without any satellite lesions, it's unlikely.  Topical steroids may help and you can search for a cause- something he rubbed on the knee or something that contacted it.
